Goals Support in Forms

Ellevation's Form Templates give your team practical tools to document, monitor, and update student goals, which keeps your EL program workflows consistent and accountable. This article covers two ways to track goals in Forms, plus how to correct contributor mistakes.

To access your district's Form Templates:

  1. Click the Admin menu icon (), then navigate to Forms Settings > Templates.
  2. Click the name of a Form Template to preview it.

Addressing Goals in Forms

  1. Search for Goals under the Add Section panel.
  2. Select Goals from the Form section type list under "Data Blocks."
  3. (Optional) Add a description.
  4. Select Academic Session, then Pick an option (Academic Year, Semester, or Quarter). Add any additional Academic Session(s) by selecting +Add Academic Session.
  5. (Optional) Configure a custom title.
  6. Click Save.

Acknowledgment

The Acknowledgment feature in Forms lets you document that a contributor has taken the appropriate action on the Goals tab of the student profile. By adding an Acknowledgment section to a Form Template, you can require contributors to confirm a specific statement, such as "I have set and/or updated the student's goals appropriately," before they can sign and submit the form.

💡 Quick Tip

Pair the Acknowledgment section with the Goals tab of the student profile. Educators can review and update goals there first, then confirm their actions directly in the progress monitoring form.

Add an Acknowledgment Section to a Template

  1. Open the Form Template and click the Acknowledgment section in the preview panel.
  2. Click Enter acknowledgment text in the configuration panel on the left, then type your acknowledgment statement in the text box.
  3. Click Add, then click Save.

ℹ️ Note

Acknowledgments are captured on the form but are not viewable on the Student List.

Forms Goal Sections

You can use Forms Goal Sections to document and track student goals directly within a Form Template. Goal Sections are independent from the Goals tab. Entries made here do not affect or update the Goals tab in the student profile.

Two section types are available: Goal sections (to document up to four individual goals) and Goal Progress sections (to track progress on up to four goals). You can also add a Descriptors review section to help contributors reference proficiency level descriptors while completing the form.

💡 Quick Tip

Goal and Goal Progress fields can also be added as columns on the Student List, giving you a quick view of goal data across your caseload without opening individual forms.

Add a Descriptors Section

  1. Click the +Add button.
  2. Search for Descriptors.
  3. Under the Review header, click Descriptors.
  4. (Optional) In the pop-up, add a description such as "Use the descriptors to track the student's progress toward their goals."
  5. Click Add.

Add a Goal and a Goal Progress Section

Goal sections document up to four individual goals, and Goal Progress sections track progress on those goals. Add each section from the Add Section panel, then configure it as a Collect Data or View Data section.

  1. Search for Goal under the Add Section panel.
  2. Select Goal 1 from the Form section type list under "Language Progress." Add Goal 2, Goal 3, and so on for additional goals.
  3. Choose Collect Data or View Data, then configure the section using the matching steps below.
  4. Repeat these steps to add Goal Progress section(s).

Configure a Collect Data section:

  1. (Optional) Add a description. We recommend a prompt such as "How is the student progressing on their goal?"
  2. (Optional) Enable Include Comment.
  3. Click to include a comment, then add the comment label "Supporting Evidence."
  4. To make contributor input mandatory, toggle the Required option (recommended).
  5. (Optional) Configure a custom title or answer options to document the student's goal specifically.
  6. Click Save.

Configure a View Data section:

  1. (Optional) Add a description.
  2. Choose the date range for the goals you want to view.
  3. Refine by the Cycle for the goals you want to view.
  4. (Optional) Configure a custom title or answer options to document the student's goal specifically.
  5. Click Save.

What If a Contributor Makes a Mistake?

Mistakes happen, and Ellevation makes them easy to address. Users with the appropriate permissions (as configured by your district's Ellevation Administrator) can reset progress on a form for individual students or in bulk. This action is available for forms in the In Progress or Done Editing buckets.

⚠️ Warning

Resetting progress clears a contributor's entries for that form. This action cannot be undone. Confirm you have selected the correct student(s) before proceeding.

Reset Progress for a Single Form

  1. Open the Cycle and click into the In Progress or Done Editing bucket.
  2. Click the icon to the right of the student's form.
  3. Click Reset Progress.

Reset Progress in Bulk

  1. Open the Cycle and click into the In Progress or Done Editing bucket.
  2. Select the students whose progress you want to reset.
  3. Click the Actions button.
  4. Click Reset Progress.
  5. Confirm by clicking the red Reset Progress button in the pop-up.
