Florida Reevaluation with Forms for Specialists!

This article walks Specialist Account Types through running a full reevaluation cycle in Ellevation, from adding students to analyzing meeting form data, so your EL program records stay current and complete.

Follow these five steps to complete a reevaluation cycle:

Step 1: Add Students to the Reevaluation Cycle

Start by adding students to the Reevaluation Cycle so their meetings can be scheduled and tracked.

  1. Navigate to your district's Reevaluation dashboard and click the month's tile containing the students you want to schedule.
  2. Select the students for whom you'd like to schedule a meeting.
  3. Click Edit.
  4. Click Add to Cycle.

  1. Select the Cycle.
  2. Select a Meeting Date (optional).
  3. Click Submit.

💡 Quick Tip

If a student needs an additional meeting during the year, add the form from within the cycle using the Add Forms button instead:

  1. Click the icon in the upper right corner of the cycle.
  2. Enter the student name(s) or student ID(s).
  3. Add the contributor's role and names.
  4. Click Add.

📝 Use Add Forms to add a single student's form mid-year.

ℹ️ Note

Add a contributor role and name for anyone who will not attend but whose signature you'd like to collect later. This creates a signature line on the form. If you know a contributor will not attend, mark them as optional.

Step 2: Manage Contributors on Meeting Forms

Next, add the contributors, or meeting attendees, to the Meeting Form. Your Forms populate contributor roles automatically, but not the names of the people in those roles. To add contributor names:

  1. Click the icon to the right of the student's row.
  2. Select Manage Contributors.
  3. Begin typing a contributor's name.
  4. Select the name to add the contributor to the form. Repeat for all contributors.

💡 Quick Tip

To add contributors to forms in bulk, follow these steps instead:

  1. Select all students by clicking the box next to the student names.
  2. Select Actions.
  3. Select Manage Contributors.
  4. Begin typing a contributor's name.
  5. Click Save.

📝 Use Actions to manage contributors for multiple students at once.

Your form may be missing the roles of contributors who are attending the meeting. For example, your form may not include the Counselor role even though the school counselor is attending. To add new contributor roles:

  1. Click the icon to the right of the student's row.
  2. Click Add Roles/Contributors.
  3. Choose the appropriate role or contributor.
  4. Click Save.

Step 3: Conduct Reevaluation Meetings

Now conduct the Reevaluation Meeting. Open the form and complete all required questions:

  1. Click the icon to the right of the student's row.
  2. Click Open Form.
  3. Fill out the form.

💡 Quick Tip

To open all your forms at once, follow these steps instead:

  1. Select all students by clicking the box next to the student names.
  2. Select Actions.
  3. Select Open Form(s).
  4. Fill out the form.

Step 4: Sign Reevaluation Meeting Forms

Next, sign the Reevaluation Meeting form. If all contributors sign from the Specialist's computer, each meeting attendee completes these steps:

  1. Click the Sign button at the bottom of the form.
  2. Select the appropriate contributor on the left, then choose Choose or Draw for the signature. If you draw the signature, click Sign As.
  3. Once all contributors have signed, click the Save and Submit button.

💡 Quick Tip

If users must sign from their own logins, have meeting attendees who use Ellevation log in to their own accounts and complete the steps above. Collect wet signatures from any attendees who do not have an Ellevation account, such as families and guardians.

To collect wet signatures, follow these steps:

  1. Submit your completed form.
  2. Go to the Submitted bucket and click the icon to the right of the student's form.
  3. Click Print Form.
  4. Click the printer icon to see your printed forms.
  5. Click the download icon to download and print your forms.

💡 Quick Tip

To print forms in bulk:

  1. Go to the Submitted bucket.
  2. Check the box next to the students whose forms you want to print.
  3. Select Actions.
  4. Select Print Forms.
  5. Click the printer icon to see your printed forms.
  6. Click the download icon to download and print your forms.

Clone the sample dashboard in your database and adjust the tiles to match your monitoring periods, quarter dates, and template questions.
