Configure Form Templates to address Classroom Modifications

Classroom Modifications help English learners (ELs) access grade-level learning without changing the standards they're working toward. This article shows you how to configure the Classroom Modifications section in Form Templates so educators can review, add, or edit those supports directly inside a form.

ℹ️ Note

This article uses the term "Classroom Modifications." Your district or state may use a different label, such as Instructional Modifications, Instructional Accommodations, Classroom Accommodations, Instructional Supports, or Modifications. If you don't see a Classroom Modifications section in the Form Template Builder, check your district's label by navigating to the Admin menu icon > Account Settings > Labels > Terms > Modifications field. Choose the Right Configuration for Your District

Before building your Form Template, decide whether educators should be able to edit Classroom Modifications inside a form or review them only. The table below maps each scenario to the right setup.

Update Existing Classroom Modifications Section

To update an existing Modifications section 

  1. Click the preview on the right (the section with the arrow in the screenshot below) to open up the edit panel on the left.
  2. Make any necessary adjustments (districts typically adjust the Academic Session each semester or annually).

Set Up a Review-Only Classroom Modifications Section

Use this configuration when educators need to reference a student's assigned Classroom Modifications while completing a form, but should not edit or update them. This keeps modifications protected in the student's profile while still surfacing them at the point of need.

To add a review-only Classroom Modifications section to a form template:

  1. Navigate to the Admin menu icon > Forms Settings > Templates.
    • To create a new template, click New.
    • To edit an existing template, click the template name.
    • For more information on form templates, see Configure Form Templates.
  2. Search for Classroom Modifications under the Add Section panel. This section may appear as  Instructional Modifications, Instructional Accommodations, Classroom Accommodations, Instructional Supports, or Modifications.
  3. Select Classroom Modifications from the Form section type list under "Determinations."
    • (Optionally) add a description. 
  4. Select the Academic Period
  5. Leave the Allow contributors to assign and edit toggle turned off.
  6. [Optional] To include Classroom Modifications from additional academic periods, click Add Academic Period.
  7. Click Save.

When contributors complete a form that includes this section, they'll see the student's Classroom Modifications for the selected academic period(s) — but won't be able to make changes.

Set Up an Editable Classroom Modifications Section

Use this configuration when educators should be able to add, edit, or delete Classroom Modifications for students directly inside a form. Ellevation supports two assignment methods: for all instructional settings or by subject area.

Add or Edit Classroom Modifications for All Instructional Settings

Classroom Modifications for all instructional settings apply a single set of supports to a student across all of their classes or instructional interactions. This is best suited for elementary students or situations where one set of modifications covers all contexts — for example, K–5 students who receive the same classroom modifications regardless of subject.

To configure this option in a form template:

  1. Navigate to the Admin menu icon > Forms Settings > Templates.
    • To create a new template, click New.
    • To edit an existing template, click the template name.
    • For more information, see Configure Form Templates.
  2. Search for Classroom Modifications under the Add Section panel. This section may appear as  Instructional Modifications, Instructional Accommodations, Classroom Accommodations, Instructional Supports, or Modifications.
  3. Select Classroom Modifications from the Form section type list under "Determinations."
    • (Optionally) add a description. 
  4. Select the Academic Period
  5. Set the Allow contributors to assign and edit toggle to on.
  6. Under How should contributors assign classroom modifications for students, select For all instructional settings.
  7. [Optional] To include Classroom Modifications from additional or previous academic periods, click Add Academic Period.
  8. Click Save.

When contributors complete the form, they'll see the student's Classroom Modifications for the selected academic period(s) in a table. If no modifications have been assigned, contributors will see "none added." They'll also be able to assign and edit Classroom Modifications for all instructional settings for the selected academic period(s).

Add or Edit Classroom Modifications by Subject Area

Classroom Modifications by subject area allow educators to assign differentiated supports based on course or content area. This is ideal for secondary students who may need different modifications in different classes — for example, unique supports for ELA versus math.

To configure this option in a form template:

  1. Navigate to the Admin menu icon > Forms Settings > Templates.
    • To create a new template, click New.
    • To edit an existing template, click the template name.
    • For more information, see Configure Form Templates.
  2. Search for Classroom Modifications under the Add Section panel. This section may appear as  Instructional Modifications, Instructional Accommodations, Classroom Accommodations, Instructional Supports, or Modifications.
  3. Select Classroom Modifications from the Form section type list under "Determinations."
    • (Optionally) add a description. 
  4. Select the Academic Period
  5. Set the Allow contributors to assign and edit toggle to on.
  6. Under How should contributors assign classroom modifications for students, select By Subject Area.
  7. [Optional] To include Classroom Modifications from additional or previous academic periods, click Add Academic Period.
  8. Click Save.

When contributors complete the form, they'll see the student's Classroom Modifications for the selected academic period(s) in a table, organized by subject. If no modifications have been assigned, contributors will see "none added." They'll also be able to assign and edit Classroom Modifications by subject area for the selected academic period(s).

View Historic Classroom Modifications Data

To review a student's past Classroom Modifications, navigate to the Classroom Modifications tab on the student's profile.

  • Modifications added before August 12, 2024 display as modifications for "All instructional settings" and appear under the Previously Assigned section.
  • Modifications assigned on or after August 12, 2024 display with the assigned academic period and indicate whether they apply to All Instructional Settings or By Subject.

💡 Quick Tip

If you need to track Classroom Modifications outside of forms — for example, to bulk assign supports across a cohort — use the Student Profile or Student List instead. See Assign Classroom Modifications/Instructional Supports for details.
