Ellevation Forms include four distinct Language Program sections, each built for a different workflow. This article helps you choose the section that fits your district's needs and set it up in your form templates.

This article applies to districts that track or assign Language Programs in Ellevation. If your district uses Services instead, refer to the Services article. If your district uses both, read this article first, then review Use Forms to Address Services.

Recommend a Language Program for a Student
The Language Program Recommendation section lets contributors specify which program a student should be placed in without directly updating the student's record in Ellevation. This section works well when your district:
- Requires family approval or another review step before assigning a program.
- Codes Language Programs in your district's SIS.
- Note: Programs coded in your SIS override any programs assigned directly in Ellevation.
Set Up a Language Program Recommendation Section
- Navigate to the Admin menu icon > Forms Settings > Templates.
- To create a new template, click New.
- To edit an existing template, click the name of the template.
- For general template guidance, review Configure Form Templates.
- Search for Language Program Recommendation under the Add Section panel.
- Select Language Program Recommendation from the Form section type list under "Determinations."
- Choose to Collect Data and make any other adjustments or configurations that might be needed.
📝 Search for and select "Language Program Recommendation" from the Add Section panel on the left.
When an educator completes a form that includes the Language Program Recommendation section, all district language programs appear as a multiple-choice list.
📝 Contributors see all district language programs as selectable options when filling out the form.
After educators submit forms, you can view recommendation responses using the Language Program Recommendation field in the Student List. You can also create a form template with a Language Program Recommendation Review section to surface this data.

💡 Quick Tip
To support SIS uploads or compare data, export your Student List view to CSV: go to Reports > Export to CSV. You can also create Student List views or data dashboards using input from forms.
Review a Student's Language Program Assignments
The Language Program Assignment Review section lets contributors view a student's current and previous language program enrollments within a form, without the ability to make changes. Use this section when your district needs educators to reference language program history as part of a workflow, such as during an annual review or program transition.
Set Up a Language Program Assignment Review Section
- Navigate to the Admin menu icon > Forms Settings > Templates.
- To create a new template, click New.
- To edit an existing template, click the name of the template.
- For general template guidance, review Configure Form Templates.
- Search for Language Program Assignment under the Add Section panel.
- Select Language Program Assignment from the Form section type list under "Determinations."
- Choose to View Data and make any other adjustments or configurations that might be needed.
- Keep the Allow contributor to add a new language program toggle turned off (grayed out) to limit for review only.
- (Optional) Add helper texts or select specific dates to limit which programs display in the form.

Assign a Language Program to a Student
The Language Program Assignment section lets contributors review a student's existing language program history and enroll them in a new program, all within a single form. This section updates the student's record in Ellevation directly upon form submission. Use this section when your district does not import language program data from your SIS.
⚠️ Warning
If your district imports language programs from your SIS, any changes made through a form will be overwritten the next time your SIS syncs. To use forms to recommend changes and export those recommendations for your SIS instead, use the Language Program Recommendation section.
Set Up a Language Program Assignment Section
- Navigate to the Admin menu icon > Forms Settings > Templates.
- To create a new template, click New.
- To edit an existing template, click the name of the template.
- For general template guidance, review Configure Form Templates.
- Search for Language Program Assignment under the Add Section panel.
- Select Language Program Assignment from the Form section type list under "Determinations."
- Choose to View Data and make any other adjustments or configurations that might be needed.
- Turn on the Allow contributor to add a new language program toggle.
- (Optional) Add a description or select specific dates to limit which programs display in the form.

Complete a Language Program Assignment in a Form
When an educator opens a form with this section, they see the student's existing language program enrollments. To add a new enrollment:
- Click + Add Language Program.
📝 Existing language program enrollments appear in the form. Click "+ Add Language Program" to enroll the student in a new program.
- Select the Language Program and a Start Date, then click Add.
📝 Select the program and start date. No changes are saved to the student's profile until the form is submitted.
- Review the updated language program assignment before submitting. Note the following:
- Added: The newly selected language program appears in the list. Click the ⋮ (three-dot) menu to remove, edit, or adjust the newly added program.
- Edited: The previous language program's end date is automatically set to the day before the new program's start date.
📝 Review the assignment before submitting. The previous program's end date updates automatically when a new program is added.
- Submit the form. Language program changes take effect immediately and update the student's Services and Programs tab upon submission.
Document a Student's Language Program History
The Language Program History section lets contributors review or add language program information from other districts or states. Use this section when a student is new to your district and you need to document their prior EL services as part of enrollment or identification, such as for:
- New Student Intake Forms.
- Beginning-of-year (BOY) or initial identification LPAC meetings.
Set Up a Language Program History Section
- Navigate to the Admin menu icon > Forms Settings > Templates.
- To create a new template, click New.
- To edit an existing template, click the name of the template.
- For general template guidance, review Configure Form Templates.
- Search for Language Program History under the Add Section panel.
- Select Language Program History from the Form section type list under "Determinations."
- Choose to Collect Data or View Data based on your needs and make any other adjustments or configurations that might be needed.
When completing the form, contributors can select multiple historical program options to build a complete picture of how the student was previously served.

After forms are submitted, you can view Language Program History responses using the Language Program History field in the Student List.
