Print, Analyze Data, and Take Action with Forms

Once educators submit their Forms, you can turn that input into Student List Views and data dashboards that surface trends and help you take action on behalf of English learners (ELs). This article walks you through how to view, filter, visualize, review, print, and export submitted Form data.

View Submitted Results

Not sure where to start? Ellevation can generate a Student List View for you โ€” pre-loaded with filters and fields based on your submitted Forms data โ€” with a single click.

  1. Navigate to the Submitted bucket within a Cycle.
  2. Click the โ‹ฎ icon at the top right of the screen.
  3. Select View Submitted Results.

๐Ÿ“ Clicking View Submitted Results from the Submitted bucket automatically generates a filtered Student List View for that Cycle.

The generated Student List View reflects only the Forms submitted from that specific Cycle. From here, you can add more data points or charts and save it as a new Student List View.

Use the Student List to Visualize Form Data

Student List Views built on Forms input let you quickly identify trends from educator responses and take action. You can filter by Cycle, refine by Form Status, add Form Fields as columns or charts, and save the view for ongoing use.

Follow these steps to create a Student List View based on Forms input:

  1. In the Student List, open Filter > Forms > Cycle.
  2. Under is any of, find and select the Cycle you want to view.
  3. Refine by Form Status or Meeting Date as needed. Form Status options include:
    • Not Started โ€” Forms assigned to students and educators in a Cycle but not yet opened.
    • In Progress โ€” Forms started by educators but not yet completed.
    • Done โ€” Forms completed by educators but not yet signed and submitted.
    • Submitted โ€” Completed Forms within the selected Cycle.
  4. Continue filtering by decisions (such as monitoring recommendations or observed reading skills) by searching for Form Fields:
    • To add columns based on Forms: click +Add Field > search Form Fields; or click +Add Field > search Form Status to add Cycle Status.
    • To add charts based on Forms: click Chart > search Form Fields.
  5. Save your view: click Save As > New Student List View > name the view > click Save View.
  6. If needed, export the data from the Student List View as a CSV. Learn how to export Student List View data.

Filter and Flag Specific Form Responses

In addition to filtering by Cycle, you can filter the Student List for specific Form Field responses and apply visual highlights to flag students who match a set of criteria. This is especially useful for quickly identifying students for whom educators have noted a need for additional support or whose progress was recorded as not meeting grade-level expectations.

To filter by a specific Form Field response, type form fields in the filters search bar and select the field you want to filter by.

Set Highlight Rules for Form Field Values

You can apply color highlight rules to visually flag students whose Form Field values match specified criteria โ€” for example, highlighting students identified by the EL Team as needing a follow-up.

To set highlight rules:

  1. Click Fields at the top of the Student List.
  2. Customize highlight rules:
    • To highlight specific data points: click the Highlight icon to the right of the field you want to configure, choose the option that represents the highlight rule you want to set, choose the highlight color, and click Save.
    • To edit an existing highlight rule: click Edit below the highlight rule you want to change, update the rule option and/or color as needed, and click Save.
    • To remove a highlight rule, click the Delete icon to the right of the rule you want to remove.
  3. Click Apply Changes.

Once your filters, fields, and highlight rules are in place, you can save the view as a Student List View or as a Dashboard Tile. Continue to the next section to learn how to build a Dashboard from Forms data.

Use Dashboards to Visualize Form Data

Data Dashboards built on Forms input give you a visual, at-a-glance view of trends across educator responses โ€” and let you take action quickly.

โ„น๏ธ Note

Only the person who created a Dashboard can add tiles to it. However, new dashboards can be created and shared with others.

  1. In the Student List, open Filter > Forms > Cycle.

  1. Click Save As > Dashboard Tile.
  2. Click Dashboard > Create New.
  3. Type a name for the Dashboard.
  4. Adjust tile settings as needed (Headline, Table, or Chart tile).
  5. Enter a name for the tile in the Name field.
  6. Click Add to Dashboard.

To add more tiles, follow the steps below to save to an existing Dashboard.

  1. Click Save As > Dashboard Tile.
  2. Click Dashboard and select the name of an existing Dashboard.
  3. Adjust tile settings as needed (Headline, Table, or Chart tile).
  4. Enter a name for the tile in the Name field.
  5. Click Add to Dashboard.

To create additional tiles, adjust your Filters and Fields as needed and repeat steps 1โ€“5.

To view your Dashboard after saving, click the Ellevation logo at the top left corner of the screen and toggle to your Dashboard from the home screen.

Review and Print Submitted Forms

After Forms have been submitted, you can review and print them from the Submitted bucket. The Print Form(s) action generates a printable PDF of any selected Forms, accessible via the printer icon on the right side of the screen.

Review or Print Multiple Submitted Forms

  1. Open the Submitted bucket in a Cycle.
  2. Click the checkboxes next to the student names you want to include, or click the checkbox next to Student to select all students.
  3. Click Actions:
    • To review only: select Open.
    • To review and print: select Print Forms, then click the printer icon and choose the Forms you want to print.

Review or Print an Individual Submitted Form

  1. Open the Submitted bucket in a Cycle.
  2. Click the checkbox next to the student's name.
  3. Click Actions:
    • To review only: select Open.
    • To review and print: select Print Forms, then click the printer icon and choose the Forms you want to print.

Review a Submitted Form on the Student Profile

  1. Click the โ‹ฎ icon to the right of the student's name.
  2. Select Open Student Profile.
  3. Click the Forms tab to view submitted Forms for that student.

Print a Student's Forms Across Multiple Cycles

The Forms tab on a student's profile lets you select forms and cycles and print them together as a single report, so you can assemble a cumulative folder or a transfer packet without downloading each form one at a time.

Select the Forms and Cycles to Print

  1. From the Student Profile, open the Forms tab. Each cycle, and each form within a cycle, has a checkbox beside it.
  2. Check the boxes for the specific cycles or individual forms you want to include in the printout.
  3. To move faster, use Select All to select every form at once, or Clear to remove all current selections.

๐Ÿ’ก Quick Tip

If you are building a cumulative folder or preparing records for a student who is transferring, use Select All to select every cycle and form at once, then generate one combined report.

Generate the Report

  1. Click Generate Report. This button stays disabled until at least one form is selected.
  2. The Reports Gallery opens in a new tab. Your student's report appears in the list, with the most recently generated report at the top.

Open the Report as a PDF

  1. In the Reports Gallery list, locate the student's report (the most recent is at the top) and click the arrow icon to open it as a PDF in a new tab.
  2. The PDF includes all of the forms you selected for that student, ordered by form creation date with the most recently created form first. Print or save the PDF from your browser's PDF viewer.

Generate a Cycle Summary

The Cycle Summary is a condensed, printable or shareable PDF of decisions made within a submitted Cycle. It reduces manual documentation work, supports audit readiness, and gives educators a streamlined experience managing compliance records. Multiple students appear per page, and the cover page includes Cycle details and contributor signatures. Only responses and decisions that were actually recorded are displayed โ€” rather than all available form options โ€” keeping the report concise and easy to scan.

Depending on your state or district, this action will appear as either Create LPAC Meeting Minutes or Create Cycle Summary โ€” the functionality is the same regardless of the label. For Texas districts, LPAC Meeting Minutes summarize all LPAC decisions and actions for a given Cycle. These reports can be printed and placed in the district or campus LPAC Binder or your district's filing system for compliance purposes. Check with your District Ellevation Administrator to confirm your district's specific policy and process.

To generate a Cycle Summary:

  1. Navigate to the Submitted bucket in a Cycle.
  2. Check the box(es) next to the desired student(s).
  3. Click Actions > Create Cycle Summary.

  1. Select the form sections you want to include in the Cycle Summary.
  2. Click Generate Cycle Summary.

Sections marked with the Collected icon indicate form fields designed to gather input from multiple contributors. Including these sections will display all contributor responses in the summary.

  1. Once the download is complete, click Open PDF to open the summary in a new tab for printing or electronic sharing.
