How Educators Complete their Assigned Forms

Completing your assigned forms in Ellevation lets you support EL progress monitoring, provide feedback for exited students, and contribute input on reclassification readiness and designated support assignments โ€” keeping your students' records current and complete.

This article covers individual form completion by educators and classroom teachers โ€” for example, progress monitoring. If you are completing forms as part of a meeting or LPAC process, contact your district's Partner Success Manager for guidance.

Access Your Assigned Forms

You may receive an email with a direct link to your assigned forms โ€” clicking that link takes you straight into your form queue. You can also access forms at any time from within Ellevation.

To access your forms from Ellevation, click the Forms tab from any page. The Forms page displays all of your assigned forms organized into Cycles. Cycles are time-based groupings of forms defined by due dates and overall completion progress.

Fill Out Assigned Forms

When you have forms to complete, a notification appears on the Forms tab in the navigation bar. Click the Forms tab to see your Cycles.

To fill out your assigned forms within a Cycle in bulk, hover over the Cycle and click Fill out forms.

You can also open forms from the Cycle detail page, which gives you a bird's-eye view of all form statuses โ€” Not Started, In Progress, and Done โ€” making it easier to track your progress and complete forms in an organized way. Click the Cycle name or click the โ‹ฎ icon and select View to open the Cycle detail page.

From the Cycle detail page, click the Fill out forms button in the top-right corner to open your queue.

Open Forms From a Status Bucket

Forms are organized into status buckets: Not Started, In Progress, and Done. To open one or more forms from a bucket:

  1. Select the Not Started bucket (or another status bucket as needed).
  2. Click the checkboxes next to individual student names, or click the checkbox next to Student to select all students.
  3. Click Actions > Open.

A queue of forms opens, with students listed alphabetically in the left sidebar. Click any student name to navigate between forms. A green checkmark appears next to a student's name when their form is complete.

Key student information is displayed at the top of each form. All questions marked Required must be answered before you can submit. If required questions remain unanswered, you will see a validation banner.

Forms are configured by the Ellevation Administrator at your school district. If you are unsure how to answer a question, reach out to your Ellevation Administrator for guidance.

Sign and Submit a Form

๐Ÿ“ If an electronic signature is required, select or draw your signature before clicking Submit.

If your form requires an electronic signature, choose or draw your signature, then click Submit. Once you are satisfied with your answers, click Submit to finalize the form.

โš ๏ธ Warning

In most cases, teachers cannot make changes to a form after it has been submitted. Review all of your responses carefully before clicking Submit.

After submitting, you will have the option to continue filling out any remaining assigned forms.

Sign and Submit Multiple Forms at Once

If you have several forms that require electronic signatures and you want to sign them all in one step, use the batch sign-and-submit workflow below.

To sign and submit multiple forms at once:

  1. Open forms individually or in bulk and answer all questions for each student.
  2. Click the Done Editing toggle instead of Sign and Submit after completing each form. Repeat this step for every form in your queue.
  3. Return to the Cycle overview and click into the Done bucket.
  4. Click the checkboxes next to student names, or click the checkbox next to Student to select all.
  5. Click Actions > Sign and Submit > select your name and contributor role > choose or draw your signature > click Sign as > Sign and Submit.

๐Ÿ“ Use the Done Editing toggle (not Sign and Submit) when you plan to batch-sign forms later.

๐Ÿ“ Select all completed forms in the Done bucket, then use Actions โ†’ Sign and Submit to sign them in one step.

After signing, your submitted forms will be available in the Submitted bucket for review.

๐Ÿ’ก Quick Tip

The batch sign-and-submit workflow is especially useful when you are the sole contributor โ€” or the last contributor in a meeting โ€” to sign a set of forms.

Mark All Forms as Done Editing

If your forms contain only review sections (no data-collection sections), you can mark all forms as Done Editing in one step rather than opening each one individually.

  1. Click the checkbox next to Student to select all students.
    • If your Cycle has more than 25 forms, click the hyperlink labeled Select all students across pages to include everyone.
  2. Click the Actions menu.
  3. Select Mark as Done Editing.

All selected forms will move into the Done Editing bucket, where you can then bulk sign and submit them.

Review Completed Forms

To access completed forms, click the Forms tab from any page in Ellevation, then click the Cycle you want to review.

Review Multiple Submitted Forms at Once

  1. Click the Submitted bucket.
  2. Click the checkboxes next to student names, or click the checkbox next to Student to select all.
  3. Click Actions > Open.

Review a Form on the Student Profile

  1. Click the โ‹ฎ icon to the right of the student's name.
  2. Select Open Student Profile.
  3. Click the Forms tab on the student's profile.

Student Still Visible After Leaving Your Roster

If a student has been promoted or transferred but you can still see their profile in an open Cycle, it is because that Cycle has not yet been closed or archived. Access is maintained as long as you are still assigned to forms in an active Cycle. Contact your district's Ellevation Administrator for assistance if a student who is no longer on your roster continues to appear.

Complete Services, Testing Accommodations, and Classroom Supports Sections

Some forms include sections for Services, Testing Accommodations, and Classroom Supports. The guidance below explains how to complete each section when it appears in a form.

Services

Edit an Existing Service in a Form

  1. Click the โ‹ฎ (three dots) icon > click Edit next to the service name.
  2. Make your edits > click Add.
    • An edited service will appear with an Edited badge in the form.
    • The service change is staged in the form only โ€” it will not update the student's profile until the form is submitted.
  3. When ready, submit the form to apply your changes.

Remove an Existing Service in a Form

  1. Click the โ‹ฎ (three dots) icon > click Remove next to the service name.
    • The service will be removed from the form table but will not update the student's profile until the form is submitted.
  2. When ready, submit the form to apply your changes.

Testing Accommodations

Add or Delete a Test in a Form

In a Cycle with an editable Testing Accommodation or Testing Accommodation Review section, you can add or delete a test before submitting. This is helpful when a student is assigned a test with accommodations they will not take, or when contributors need to add accommodations for a test not yet assigned.

To add a test:

  1. Select Add New Test from the appropriate form section.
  2. Select the test and academic period, then click Next.
  3. Choose the appropriate accommodations and click Save.

To delete a test:

  1. Click the ellipsis (โ€ฆ) to the right of the test, then select Delete Test.
  2. In the confirmation modal, click Delete.

Add or Edit Classroom Modifications for All Instructional Settings

When no modifications have been added yet:

  1. Click Assign > choose the classroom modifications the student should have from the list.
  2. Click Save.
  3. Any new additions will appear with an Added tag. Click Edit to make changes before submitting.

When modifications already exist:

  1. Click Edit > check or uncheck the relevant modifications from the list.
  2. Click Save.
  3. Any changes will appear with an Edited tag.

After the form is submitted, the student's profile will be updated to reflect any changes.

Classroom Supports

Add or Edit Classroom Modifications by Subject Area

When no modifications have been added yet:

  1. Find the subject row you want to update and click Assign.
  2. Choose the modifications the student should have from the list.
  3. Click Save.
  4. New additions will appear with an Added tag.

  1. Find the subject row you want to update and click Edit.
