Review Documents in Forms

Adding a Document Review section to a Forms template lets educators view relevant student documents directly within a form. This creates a clear record of the documentation used to inform student decisions and supports your compliance workflows and audits.

ℹ️ Note

Documents must be uploaded in Ellevation before they can be added to a Forms template for review. See Learn About and Upload Student Documents for instructions.

Access Your Forms Templates

To reach your Forms templates, navigate to the Admin menu icon, then select Forms Settings > Templates.

From the Templates list, you can either edit an existing template or create a new one:

  • To edit an existing template: Click the icon next to the template you want to edit.
  • To create a new template: Click New Template, enter a name in the Form Template field, and click OK. The Form Template Builder will open automatically.

Add a Document Review Section

Once you are in the Form Template Builder, follow these steps to add a Document Review section:

  1. Search for Document Review under the Add Section panel. 
  2. Select Document Review from the Form section type list under "Student Information."
  3. (Optional) Add a description.
  4. Select document type(s) from the drop down menu.
    • [Optional] Add a description.
  5. Click Save.

💡 Quick Tip

You can add multiple Document Review sections to a single template — one for each document type your workflow requires. This keeps related documentation organized and easy for reviewers to locate during form completion.

How Document Review Appears During Form Completion

Once the template is saved, the Document Review section will be visible to educators when completing the form. Relevant uploaded documents appear inline, giving reviewers immediate access to supporting materials without leaving the form.
