Configure Form Templates to address Testing Accommodations

Testing Accommodations are the specific resources a student receives for one or more standardized tests, such as the use of a dictionary. Use Ellevation's Form Template Builder to let educators review, add, edit, recommend, or confirm Testing Accommodations directly within a form, keeping student records accurate and up to date.

ℹ️ Note

Testing Accommodations may appear under a different label in your district. Common alternatives include Accommodations, Testing Modifications, and Designated Supports. If you don't see a Testing Accommodation section in the Form Template Builder, check your label settings: navigate to the Admin menu icon > Account Settings > Labels > Terms. Choose the Right Testing Accommodations Section for Your Form

Ellevation's Form Template Builder offers multiple Testing Accommodations sections, each designed for a different workflow. Use the table below to identify which section fits your district's goal before building your template.

Review and/or Update Testing Accommodations

Adding or reviewing an Accommodations section to a form template lets educators see each student's current Testing Accommodations as they complete the form. Accommodations display per test, because students may have different accommodations for different assessments, for example, using a dictionary for ELA but not for Math. When you turn on the Allow contributor to assign and edit toggle, educators can also edit, add, or delete accommodations directly in the form, and submitted changes immediately update the student's profile in Ellevation.

Access the Form Template

Navigate to the Admin menu icon > Forms Settings > Templates.

  • To create a new template, click New.
  • To edit an existing template, click the name of your template.

Update Existing Accommodations Section

To update an existing accommodations section 

  1. Click the preview on the right (the section with the arrow in the screenshot below) to open up the edit panel on the left.
  2. Make any necessary adjustments (districts typically adjust the Test Year annually).

Add the Accommodations Section

  1. Search for Testing Accommodations under the Add Section panel. This section may appear as Accommodations, Testing Modifications, and Designated Supports.
  2. Select Testing Accommodations from the Form section type list under "Determinations."
  3. Choose to View Data and make any other adjustments or configurations that might be needed.
  4. If you want contributors to add new or edit existing Testing Accommodations, turn on the Allow contributor to assign and edit toggle.
  5. (Optional) Add a description and select specific academic sessions to limit which accommodations display in the form.
  6. (Optional) Select +Add Test Year to add additional academic sessions.
  7. Click Save.

💡 Quick Tip

If the toggle is on, contributors can edit, add, or delete Testing Accommodations in the live form. They use the icon to edit or delete, and the Add New Test button to add a test. Submitted edits update the student's profile in Ellevation. If the toggle is off, educators can view current accommodations but cannot make changes. What Educators See: Toggle On (Edit Enabled)

When the toggle is on, educators can add, edit, or delete a test from the Testing Accommodations section of a live form. The GIF below shows this experience.

What Contributors See: Toggle Off (Review Only)

When the toggle is off, contributors can review a student's current Testing Accommodations but cannot make changes. The image below shows this read-only view.

Add Testing Accommodations

The Testing Accommodations section under the Form category lets educators assign specific accommodations per test for individual students. Because students may have different accommodations for different assessments, accommodations are listed and assigned per test. Submitted forms update the student's profile in Ellevation unless you toggle the Collected responses must be reviewed option.

Access the Form Template

Navigate to the Admin menu icon > Forms Settings > Templates.

  • To create a new template, click New.
  • To edit an existing template, click the name of your template.

Add the Testing Accommodations Section

  1. Search for Testing Accommodations under the Add Section panel. This section may appear as Accommodations, Testing Modifications, and Designated Supports.
  2. Select Testing Accommodations from the Form section type list under "Determinations."
  3. Choose to Collect Data, choose the Academic Session, and Select Test. (Optional) Add a description.
  4. To make contributor input mandatory, toggle the Required option.
  5. To collect input without updating student profiles, select Collected responses must be reviewed. When you select this option, submitted forms will not update student information in Ellevation.
  6. Click Save.

⚠️ Warning

If Collected responses must be reviewed is not selected, submitted form information will immediately update the student's profile in Ellevation. Confirm your intent before publishing the form template.

ℹ️ Note

When Collected responses must be reviewed is selected, submitted responses are not finalized automatically. Review and finalize them in the Review and Finalize Testing Accommodation Recommendations section before they are written to the student's profile. Texas districts commonly complete this review step as part of the LPAC form.

What Contributors See When Completing the Form

When contributors open a form that includes a Testing Accommodations section, they see a list of available accommodations to assign per test. They can select one or more accommodations for each student and add optional comments.

Recommend Testing Accommodations

The Teacher Testing Accommodations Recommendation section collects input from contributors on whether students need testing accommodations or updates to their current accommodations. Because this section captures recommendations only, submitted forms do not update student profiles in Ellevation. You can review submitted responses in Forms or using the Student List.

  • To edit an existing template, click the name of your template.

Add the Teacher Testing Accommodations Recommendation Section

  1. Search for Teacher Testing Accommodations Recommendation under the Add Section panel. This section may appear as Accommodations, Testing Modifications, and Designated Supports.
  2. Select Teacher Testing Accommodations Recommendation from the Form section type list under "Determinations."
  3. Choose to Collect Data.
    • [Optional] Enable Include Comment.
  4. To make contributor input mandatory, toggle the Required option.
  5. [Optional] Make any necessary customizations to the answer options.
  6. Click Save.

💡 Quick Tip

To review previously submitted recommendation responses, add the Teacher Testing Accommodations Recommendation section under the View Data category instead. This is useful when building approval or follow-up forms that reference earlier responses.

Review and Finalize Testing Accommodation Recommendations

The Recommendation Review section lets you review the testing accommodation recommendations collected through other form sections and finalize them for each student. Texas districts commonly use this section as part of the LPAC (Language Proficiency Assessment Committee) form.

The Recommendation Review section presents two sides so you can compare what was recommended against what you decide to finalize:

  • The first column lists each student's current accommodation recommendations collected from teachers, along with any accommodations previously assigned to that student.
  • The decision side shows the accommodations that will be finalized for the student when the form is submitted.

When the form is submitted, each finalized accommodation is written to the student's profile in Ellevation.

ℹ️ Note

The Recommendation Review section reviews recommendations that were collected through other form sections. If no recommendations have been collected for a student, there is nothing to review or finalize for that student in this section.

Confirm Testing Accommodations

The Testing Accommodations Confirmation section collects input from contributors on whether students will receive testing accommodations on one or more standardized tests. You can review submitted responses in Forms or using the Student List.

Add the Testing Accommodations Confirmation Section

  1. Search for Testing Accommodations Confirmation under the Add Section panel. This section may appear as Accommodations, Testing Modifications, and Designated Supports.
  2. Select Testing Accommodations Confirmation from the Form section type list under "Determinations."
  3. Choose to Collect Data.
    1. [Optional] Enable Include Comment.
  4. To make contributor input mandatory, toggle the Required option.
  5. [Optional] Make any necessary customizations to the answer options.
  6. Click Save.

💡 Quick Tip

To review previously submitted confirmation responses, add the Testing Accommodations Confirmation section under the View Data category. This is useful when you need to audit or reference prior confirmations within a follow-up form.

Understand Testing Accommodation Eligibility (Georgia Districts)

The Testing Accommodation Eligibility section is designed specifically for Georgia districts. It helps contributors document and track the reasons why individual students are eligible for testing accommodations. Contributors can select multiple options to build a historical record in the student's profile. You can review submitted responses in Forms or using the Student List.

Access the Form Template

Navigate to the Admin menu icon > Forms Settings > Templates.

  • To create a new template, click New.
  • To edit an existing template, click the name of your template.

Add the Testing Accommodation Eligibility Section

  1. Search for Testing Accommodation Eligibility under the Add Section panel. This section may appear as Accommodations, Testing Modifications, and Designated Supports.
  2. Select Testing Accommodation Eligibility from the Form section type list under "Determinations."
  3. Choose to Collect Data.
    1. [Optional] Enable Include Comment.
  4. To make contributor input mandatory, toggle the Required option and select the minimum number of options a user must select in this section before they can submit the form.
  5. [Optional] Make any necessary customizations to the answer options.
  6. Click Save.

💡 Quick Tip

To review previously submitted eligibility responses, add the Testing Accommodation Eligibility section under the View Data category. Contributors can select multiple eligibility reasons per student, making this section useful for building a longitudinal record over time.
